If you have taken a course with us in the past two years and have lost your card, we can replace it.  We need your name and phone number and mailing address before we can re-issue your card.  DO NOT pay for the card here until you have verified with us that we have you on record.  This is the option if you want your card mailed, There is NO REFUND if you pay online here and we do not have you on record.
